Making Ozonated Oil Protocol

How manufacturers make ozonated oil: oil type, oxygen flow, ozone strength, time, storage and safety. Reference only.

Quick answer: Buy lab-tested ozonated oil instead of making your own. Manufacturers make it by bubbling ozone through plant oil, such as olive or sunflower oil, to a target strength of 200–1,200 PV. Studies used gas flows of 0.5–4 L/min and times from 15 minutes to 3 weeks. Store it in amber glass at 4 °C.

Based on the Madrid Declaration on Ozone Therapy, 4th edition (ISCO3, 2025), dental section 2.6, section 13.2 and Table 4B, and on published preparation studies

Safety. Run the outlet through an ozone destruct. Ventilate the room. Never breathe the off-gas.

What are the basics of making ozonated oil?

  • Ozonated oil is made by bubbling ozone-oxygen gas through a plant oil until the ozone has reacted with the oil's unsaturated fats. The oil turns from a greenish liquid to a paler, thicker gel. Thick or gel-like oil is not a sign of quality.
  • Strength is measured by peroxide value (PV, in mEqO2/kg), not in µg/mL. Madrid's levels are low 200 to 400, medium 400 to 600 and high 800 to 1,200. Around 1,800 it is irritating.
  • Oil: olive oil is the most used in studies. Sunflower oil takes up more ozone. Coconut oil is mostly saturated and holds fewer ozonides.
  • Oxygen: medical oxygen from a tank, or an oxygen concentrator. A concentrator is fine for making oil. Do not feed the generator room air.
  • Time has no fixed answer. It depends on the ozone concentration and flow. Studies ran from 15 minutes per 100 mL to 3 weeks, so time alone does not tell you the strength.
  • You cannot know the PV without a lab test. Madrid names the method: the European Pharmacopoeia titration, modified by Zanardi and standardized by ISCO3.

What are the best tips for making ozonated oil?

  • Use a glass vessel and a fine diffuser. Put a check valve between the vessel and the generator so oil cannot flow back.
  • Keep the oil cool. The reaction gives off heat, and heat can create harmful compounds. One study held the oil in a 30 °C water bath.
  • Label every batch with the date and the oil used. Store in amber glass at about 4 °C. Our oil article gives 6 to 12 months refrigerated.
  • Because you do not know the PV, test a small skin patch first and use it less often than you would a labeled oil.

What are the precautions for making ozonated oil, and who should avoid it?

  • Ozone destruct on the outlet, a well-ventilated room, and never breathe the off-gas.
  • Never seal the vessel. The gas outlet must stay open to the destruct. Keep flames and sparks away: Madrid names a risk of explosion.
  • Every part that touches ozone must be ozone-resistant, such as glass, silicone, stainless steel or PTFE.
  • Because by-products cannot be ruled out, use home-made oil on intact skin only. Do not swallow it, use it in the mouth, or put it inside the body.

Contraindications

Check with your healthcare provider before starting, especially if you are pregnant, have a heart or bleeding condition, or take blood thinners.

More detail

Equipment

  • An ozone generator with an ozone-resistant gas path (quartz glass, titanium, silicone). Madrid advises against using a medical generator for this.
  • Medical oxygen tank and regulator, or an oxygen concentrator
  • Glass oil bubbler with a diffusion stone and a check valve
  • Ozone destruct on the outlet
  • Amber glass bottles

What the studies used

Evidence is thin: studies rarely print gas concentration, flow and oil volume together, and none links a stated PV to a clinical result. Studies printed different units, so the settings are listed as printed and are not Madrid dosing.

  • In a lab study, 250 mL of olive oil bubbled with 80 µg/mL ozone at 4 L/min reached PV 184 after 6 hours and 224 after 12 hours, and killed more bacteria from PV 184 up (Microorganisms 2025).
  • In 123 adults with diabetic foot ulcers, olive oil bought from a producer (made with a medical generator at 50 g/m³ rather than by the authors), checked weekly for PV and stored dark at 4 °C, improved wound scores more than standard care alone (Vahedi 2025).
  • A pilot of 35 people with hard-to-heal wounds used sunflower oil made in-house with a generator on an oxygen concentrator and an ozone destruct (PV 266.5). Pathogens cleared in 13 of 17 culture-positive patients on the oil vs 1 of 7 on conventional dressings, with no test printed (Serra 2025).
