Minor Autohemotherapy (MiAH) Protocol

Concentration levels, steps, frequency and precautions for minor autohemotherapy, a small blood draw mixed with ozone and injected into the gluteal muscle.

Quick answer: Minor autohemotherapy mixes a small blood draw of 5 mL with 5 mL of ozone gas and injects it into the gluteal muscle. Concentration runs 5–40 µg/mL, given once a week. It is a practitioner protocol.

Not the same as IV ozone (MaH). Minor autohemotherapy uses 5 mL of blood given as a shot into the muscle; MaH uses 200 mL of blood given back through an IV.

What are the basics of minor autohemotherapy?

  • Minor autohemotherapy (MiAH) mixes 5 mL of the patient's own venous blood, drawn without anticoagulant, with 5 mL of ozone-oxygen gas in a 20 mL syringe. Shake gently for 1 minute, then inject the whole contents slowly into the gluteal muscle.
  • Concentration runs from 10 to 40 µg/mL in Madrid's method text, most often 20. Madrid's Table 1B groups it as low (5 to 10), medium (15 to 20) and high (30 to 40).
  • A cycle is 5 to 10 treatments, once a week.
  • Madrid describes MiAH as an immune stimulus comparable to an autovaccine, and names it the method of choice among the ozone routes for skin conditions. It also lists allergies and use as an add-on in chronic debilitating conditions.

What dose and schedule are used for minor autohemotherapy?

LevelConcentrationGas volumeOzone dose
Low5–10 µg/mL5 mL25–50 µg
Medium15–20 µg/mL5 mL75–100 µg
High30–40 µg/mL5 mL150–200 µg

Titrate: start low and increase gradually. Madrid's dermatology section shows one stepped schedule. Start at 20 µg/mL and add 5 µg/mL each session until 40 µg/mL is reached, then hold. Milder cases get one session a week for 10 weeks. Moderate to severe cases get two sessions a week for 5 weeks, then one a week for 5 weeks. Follow-up sessions every month or two are used in some cases.

What are the best tips when using minor autohemotherapy?

  • No anticoagulant is used in MiAH.
  • Fill the syringe with the gas first, then draw the blood into it through a butterfly needle from the cubital vein.
  • Inject slowly, and give the gas along with the blood.
  • Use ozone-resistant, phthalate-free syringes and medical-grade oxygen.
  • Madrid's base rules apply: first do no harm, and titrate the dose from low.

What are the precautions for minor autohemotherapy, and who should avoid it?

  • Blood is collected and handled under sterile technique. Madrid says most adverse events come from malpractice, not from the method itself.
  • Never give ozone gas alone by direct IV injection. Madrid classes it as a route that should not be used under any circumstances.

Contraindications

The Madrid Declaration lists these contraindications for systemic ozone:

  • Direct inhalation of ozone gas. The fluid lining the lungs has no antioxidants to buffer it, so inhaled ozone is toxic.
  • Glucose-6-phosphate dehydrogenase (G6PD) deficiency (favism, acute hemolytic anemia). Madrid lists it as a contraindication and recommends testing, but whether it is absolute is debated. MediSkill protocols start low and go slow; see the contraindications article.
  • Toxic hyperthyroidism (severe Graves' disease)
  • Thrombocytopenia (platelets under 50,000) and severe coagulation disorders
  • Severe cardiovascular instability
  • Acute alcohol intoxication
  • Recent acute myocardial infarction
  • Acute massive hemorrhage
  • Convulsive states
  • Hemochromatosis
  • Patients receiving intravenous copper or iron therapy

Pregnancy. Madrid advises avoiding ozone therapy in the first trimester (weeks 0 to 13) and reserves its use in pregnancy for specialists in gynecology and obstetrics.

More detail

Steps

  1. Fill a sterile 20 mL syringe with 5 mL of ozone-oxygen gas at the chosen concentration.
  2. Using a butterfly needle, draw 5 mL of blood from the cubital vein into the same syringe, with no anticoagulant.
  3. Shake gently for 1 minute.
  4. Inject the contents, blood and gas, slowly into the gluteal muscle.

What the studies used

MiAH is rarely studied alone. Most trials combine it with major autohemotherapy or other routes. Study doses are listed as printed and are not Madrid dosing.

  • In a randomized phase 1/2 trial in mild to moderate COVID-19, 7 sessions of 2 to 3 mL blood with 5 mL of gas at 25 µg/mL cleared the virus on PCR at day 10 in 100% of people vs 70% on standard care; no adverse events in the ozone group (Shah 2021).
  • In 47 women with refractory granulomatous mastitis, ozone given with steroids (MiAH at 10 to 40 µg/mL together with MAH) gave complete response at 4 months in 37.5% vs 0% on steroids alone (Cabioglu 2023).
  • In 60 people with type 2 diabetes, MAH plus MiAH twice a week for 10 weeks gave a slightly shorter visual-pathway latency than the comparison group, with no adverse events reported (Izadi 2023).
